Complete all required sections below. Your application will be reviewed by the Weatherly Scholarship Team. After submission, your recommenders will receive instructions for submitting their letters of recommendation.

You must maintain a permanent address in Cooke County, Texas to be eligible. Applicants from bordering counties may be considered at the Team's discretion.

High School Information *

Academic Performance *

If you have not taken the ACT or SAT, you may leave those fields blank.

List any academic honors, awards, scholarships, honor society memberships (such as National Honor Society), academic competitions, or other academic achievements you earned in high school. If none, write "None." (2000 character limit)

Section 3: College Enrollment Plans

Please describe your academic and career goals. What do you hope to achieve through your education? How will this scholarship help you reach these goals? (2000 character limit)

Section 4: Church Involvement and Attendance

This section represents up to 30 points of your total score (15 points for involvement + 15 points for attendance).

Church Attendance *

Describe your involvement in your church. This may include volunteer roles such as VBS helper, ministry volunteer, small group leader, worship team member, youth group participation, mission trips, or other service. Please describe your roles, responsibilities, and how your faith and church involvement have shaped your character and goals. (2000 character limit)

Tip: Be specific about your roles and the time you've committed. The Team is looking for genuine involvement, not just attendance.

Section 5: School Activities

This section represents up to 15 points of your total score.

Describe your involvement in school activities. This includes extracurriculars such as clubs, sports teams, band, choir, student government, honor societies, academic teams, theater, or other school-sponsored activities. Please describe any leadership roles you held and explain how these activities helped you grow as a person and leader. (2000 character limit)

Tip: Include specific activities, years of participation, positions held, and what you learned. Quality of involvement matters more than quantity.

Section 6: Community Service

This section represents up to 15 points of your total score.

Describe your community service activities outside of school. This may include volunteering as a tutor or reading buddy, serving with local nonprofits, community organizations, civic groups, food banks, animal shelters, or other community service. Explain how this volunteer work has impacted you and your community. (2000 character limit)

Tip: Focus on non-school, non-church activities. Describe your service, time commitment, and personal growth from these experiences.

Section 7: Financial Need

This section represents up to 40 points of your total score and is the most heavily weighted criterion.

Cost of Attendance *

Include only tuition and mandatory fees. Do NOT include room, board, books, or other expenses. The Team will verify these amounts with your institution.

Other Financial Aid *

List all scholarships, grants, loans, and other financial aid you have received or applied for. Include the source and amount (or expected amount) for each. (2000 character limit)

Athletic Status *

Student Aid Report (SAR) *

This number appears on your Student Aid Report and indicates your expected family contribution

Section 8: Letters of Recommendation

You must provide contact information for three individuals who will submit letters of recommendation on your behalf. After you submit this application, the Scholarship Coordinator will send each person a unique link to submit their recommendation.

Recommendation 3: Character and Community Reference *

This person should be able to speak to your character, academic achievement, and community involvement. This cannot be a family member.

After submitting, you will receive a confirmation email. Your recommenders will also receive instructions for submitting their letters.
